Many pragmatic trials are also restricted by the need to enroll participants on time. Some pragmatic trials also lack controls to ensure that observed differences aren't due to biases in the trial.

The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-labeled themselves as pragmatic and that were published until 2022. The PRECIS-2 tool was employed to assess pragmatism. It includes domains such as eligibility criteria, recruitment flexibility and adherence to intervention and follow-up. They discovered that 14 of these trials scored as highly or pragmatic pragmatic (i.e., scoring 5 or higher) in any one or more of these domains, and that the majority of these were single-center.

Trials with a high pragmatism rating tend to have broader eligibility criteria than traditional RCTs, which include very specific criteria that are not likely to be used in the clinical environment, and they include populations from a wide variety of hospitals. These characteristics, according to the authors, could make pragmatic trials more relevant and relevant to everyday practice. However, they don't ensure that a study is free of bias. The pragmatism is not a fixed attribute the test that doesn't have all the characteristics of an explicative study may still yield valuable and valid results.
