Item upgrades are a way to increase the stats of an item. Depending on the upgrade type it can increase an item's Mark Level or Rarity, and also boost other stats.

Item Level

When deciding whether to purchase or sell gear, players can compare the levels of the item. It's a measure of the item's condition and quality. Comparing items from a vendor's stock can be done using the item level.
The level of the item can be viewed by pressing the ALT key on PC or using the left stick (L3) on XB1 and PS4. The item will be displayed with its full description, including the ilvl. It can also be located by hovering over an item with the mouse cursor, and then typing /itemlevel into chat.
There are several theories about how the level of an item is determined. Item Stats
Item stats can make or break an item for players. The game utilizes them in various ways to encourage players not to restrict themselves to a particular weapon and to motivate players to explore other weapons. The stat mods on an weapon increase the base stats. These stat mods are then increased by a player's level.
The upgrades to an item can boost its stats. The costs for upgrades are determined by the rarity of the item. A grey or white item for instance, will only be upgraded one time, while an item that has rare tiers can be upgraded several times at more expensive costs.
Another way to change the stats of an item is to use affixes. These are special stat boosts that can be added to an item by the player and come in a variety of sizes and shapes. Affixes can do things like give the item extra damage, or give a specific ability to the item (such as reviving the dead or cutting down the duration of cooldowns) or make the item unique by giving an additional name to the item.
Currently, all affixes can be changed by selecting them from the upgrade window and pressing the green button using the arrows that circle. This will cost a certain amount of gold and other items. They can also be used as upgrade gauges as with other methods of rerolling.
In addition to Rerolling affixes and rerolls, players can select which stats to be kept on an item after it is upgraded. The possible stat ranges for an item are based on the level of the Best item upgrade, but all items are guaranteed to have at least 1 stat within the range, unless it's an exclusive item or an item that is a God roll from a raid boss.
This is another thing that could be altered by adding an option to upgrade the equipment without rolling the affixes or deciding which stats to increase. Similar to the base equipment, it would have to be done per item, and the item comparison interface will not show all possible stats simultaneously.
Item Luck
A player's Luck stat stacks up to increase the odds of finding items and the properties that are spawned from loot drops, and the intensity of these properties. However, it doesn't directly affect damage or ability to upgrade items. It merely gives players the chance to upgrade their equipment earlier in the game, and is therefore more beneficial in the initial game run as opposed to later ones, because most players value more powerful weapon scaling and defenses over raw damage gains.
Luck can also boost the chances of upgrading items at the Blacksmith. This isn't to be confused with the Blacksmith’s overall chance of upgrading, which increase with each item upgrader kit level. These can also be enhanced by using anti-materials like the Scroll of Strength and a Jewel of Blessing.
Armor, shields and weapons can be created or spawned with the Luck property, which has a default value of 40. These items can be improved with golden ingots, or spined leather (depending on their material type) for a further +40 luck or to boost the Luck of an Best item upgrader up to 140.
Additionally some items, such as the Hollow infused weapon, can be equipped to increase the damage dealt by any damage-dealing skill by 5percent. These items can be bought at the store, or they can be constructed using the right ingredients.
Item Failure
Upgrades to items can be a costly process and not all upgrades will be successful. However, there are cash shop items that allow players to boost their success rate or prevent them from losing an item if the upgrade is unsuccessful.
The rate of success of an upgrade is shown in the Honing UI. The percentage is the chances of completing the upgrade successfully and can be increased by adding Additional Materials or Special Materials. There are limitations on the extent to which success rates can be increased and the probability of an upgrade failing cannot be greater than 100%..
The Luck of an item may affect the success of an upgrade. All items have an average success rate of 25%, and every item with Luck increases this chance by 5%.
